What a Clinical Trichologist Wants You to Know About Your Scalp

The Work Behind Scalp Health

Q: Can you tell us a little about your work as a trichologist and what drew you to scalp health?

Brandy Lessafre:
As a Clinical Trichologist at NHLMA, my work focuses on identifying and treating the underlying causes of hair loss and scalp dysfunction. Hair growth is not just about the follicle itself — it’s influenced by systemic health, inflammation, hormones, nutrient status, and the condition of the scalp environment.

What initially drew me to this field was seeing how often hair loss was dismissed as purely cosmetic. In reality, it can be an early signal of deeper biological imbalances. Over the years, I’ve worked with clients of all ages experiencing a wide range of conditions, from androgenetic alopecia and telogen effluvium to inflammatory scalp disorders and microbiome disruptions.

Our approach at NHLMA is very much an “inside-out” model. We evaluate internal drivers such as metabolic health, hormones, and micronutrients while also restoring the scalp ecosystem so follicles have the healthiest possible environment to grow.

The Biggest Misconception About the Scalp

Q: What’s the biggest misconception people have about scalp health?

Brandy Lessafre:
The biggest misconception is that the scalp is simply skin with hair growing out of it. In reality, the scalp is a highly specialized biological environment that houses thousands of follicles, sebaceous glands, immune cells, and a complex microbiome.

When the scalp ecosystem becomes disrupted — whether through inflammation, microbiome imbalance, harsh products, or stress — it can directly affect the hair growth cycle. Many people focus only on stimulating hair growth, but if the scalp environment is unhealthy, the follicle simply cannot function optimally.

Healthy hair truly starts with a balanced scalp.

What She’s Seeing in Clients Today

Q: What are the most common scalp concerns you see in your clients today?

Brandy Lessafre:
One of the most common issues I see today is scalp inflammation. Many people are experiencing irritation, sensitivity, excessive shedding, or changes in hair density, and a significant portion of that is driven by inflammatory pathways.

Another major concern is microbiome imbalance. Overuse of harsh cleansers, environmental stressors, and even lifestyle factors can disrupt the natural microbial balance of the scalp. When that happens, we often see increased shedding, itching, buildup, and compromised barrier function.

I also see a growing number of clients dealing with hair loss related to metabolic health, stress physiology, and hormonal changes. Hair is a very sensitive biomarker of overall health, and the scalp often reflects those internal shifts.

Why OCHRE

Q: What made you decide to carry OCHRE in your practice?

Brandy Lessafre:
When selecting products for our clients, we are very particular about the science behind them.
